NEED TO TAKE A LEAVE OF ABSENCE?

Family Medical Leave Act (FMLA), Oregon Family Leave Act (OFLA), and/or Paid Family Medical Leave (PFML)/Paid Leave Oregon (PLO) may be an option for you. 


requesting protected leave is a three step process:

Step 1: Notify Human Resources

You must provide your employer with a notice of intent to use protected leave benefits at least 30 days in advance of your need for leave (as required by the Oregon Employment Department).

  • In the instance of an emergency/unforeseen circumstance, you need to contact HR within 24 hours of the event and submit the above form within 3 days of starting the leave.

Step 2: Complete required leave documentation

Human Resources will reach out to you within 5 days of submitting your notification (step 1) and provide required paperwork:

  • Medical Leave Request (complete and return as soon as possible).

  • Certification of Health Care Provider (complete the employee portion and give to your medical professional).

Step 3: Return Medical Certification

It is your responsibility to make sure that the Certification of Health Care Provider document is returned to HR.

  • Some medical providers will fax the form directly to HR.

  • Kaiser will put the form into your employee portal; you will need to download and share with HR.

OPTIONAL Step 4: Initiate a Paid Leave Oregon claim with The Standard

Accessing paid leave through The Standard is an optional benefit for eligible employees.

  • Call The Standard’s services center: 866-756-8115  (Mon-Fri, 5am-5pm).

  • Reference: Oregon City School District #62, Policy # 762217.

  • Complete the paperwork that is sent from The Standard and return it directly to them.

Leave qualifications

FMLA

  • Worked for employer for at least 12 months;

  • Has at least 1,250 hours of service for the employer during the 12-month period immediately preceding the leave

OFLA

  • Worked for employer for at least 180 days;

  • Has worked an average of 25 hours per week for 180 days immediately preceding the leave (excluding parental leave)

PFML/PLO

  • Work in Oregon;

  • Earned at least $1,000 during the 12-month period immediately preceding the leave

When can i take protected medical leave?

  • Own serious health condition*

  • Caring for someone with a serious health condition* (Spouse/Domestic Partner, Son/Daughter, Mother/Father, Grandparent/Grandchild)

  • Parental Leave / Pregnancy Disability

  • Taking custody or Placement of an Adopted or Foster Child

  • Sick Child Leave* (OFLA only)

  • Extended bereavement (OFLA only)

  • Bonding with child (PFML/PLO only)

  • Seeking help for yourself or a minor dependent, related to domestic violence, harassment, sexual assault, or stalking (PFML/PLO only)

  • You can take up to 12 weeks (60 days) in a 1-year period.

  • -You can take protected medical leave as Continuous or Intermittent.

    -For FMLA/OFLA, intermittent leave can be taken in full days or partial days.

  • -These are not paid leaves but we can use your accrued leave to keep you in a paid status.

    -For paid leave, file a claim for PLO with The Standard.

  • Your job and benefits are protected while you are on an approved FMLA/OFLA leave.

  • You need to apply for a leave at least 30 days before taking it. In an emergency situation, you must contact HR within 24 hours of the event and submit written notice within 3 days of starting the leave.
